Payout hold log and escalation letter

A practical log and letter template to help a small business document a payout hold, request a clear explanation and human review, and escalate in an orderly way. It is a record-keeping and communication tool, not legal advice; local rules and deadlines vary.

Subject: Request for review and release timeline — [account ID / payout reference]

Hello [support, complaints, or review team],

I am writing about a payout hold on my account, [account ID]. I expected [amount and currency] for [payout date or period]. As of [date], [amount and currency / amount not confirmed] appears to be held. The relevant payout or case reference is [reference number, if available].

The notice I received on [date] said: “[short exact quotation, if useful].” I understand that [briefly state any action or information the notice requests]. I have [completed that action on date / not received a clear list of what is needed].

Please confirm in writing:

  1. the exact amount and transactions currently affected, and whether any other payouts are affected;
  2. the specific reason for the hold and the applicable account or transaction information supporting that reason;
  3. what information or action you need from me, if anything, and how I can provide it securely;
  4. whether the decision or hold can be reviewed by a person, and how to request that review;
  5. the expected date for the review decision and, if the hold remains, the next review date; and
  6. the conditions and expected timing for release of any amount that is no longer subject to the hold.

I have attached [brief list of relevant records]. Please keep this matter under case number [case number] and tell me if it has been assigned a different reference. I would appreciate an acknowledgement and a clear next update by [date you are requesting]. If that date cannot be met, please tell me why and provide a revised date.

[Optional: The delay is affecting [specific business obligation or operational impact]. I am not asking you to disregard a valid review; I am asking for a clear explanation, a way to respond, and a stated timeline.]
